INSURANCE

1) LIABILITY INSURANCE

This insurance covers clubs use of facilities. For example if you are borrowing a school gym for some winter training then IYSA can issue a certificate of insurance that will cover you for damages. It is also common for businesses and local goverment to ask for certificates of insurance to cover practice locations. This service is completely free for members. If you require a certificate of insuance then please click on the link below to fill in the form and return it. If you do not obtain a certificate of insurance for a location and damage ocurs then you will not be covered.

2) MEDICAL INSURANCE

All IYSA registered players are covered with a secondary insurance policy. In order to be covered you need to submit the insurance form to IYSA within 90 days of the accident and seek medical attention within 60 days. The policy is good for two years from the time of the accident but you need to file quickly in order for it to be covered.

The process is as follows

  1. go to the following link and complete the insurance form. https://www.bollingersoccer.com/site/Claimsv1/default.aspx?id=37
  2. Print the completed form
  3. Submit the form to IYSA
  4. IYSA will then verify the information and submit it to the insurance company
  5. The insurance company will then issue you a claim number. At this point IYSA is out of the loop due to HIPPA rules and regulations
  6. You can then begin submitting bills to the insurance company
