Affiliate / Club Contact Form For use when changing club, or key contact information
2012-2013 Age Chart For determing age group, based on birth date
Coach / Manager add/drop For adding coaches and managers to teams or for running volunteer background check
Roster Form Submitting a team via Excel spreadhseet which is an alternative to using online registration
Fee Dues Form A simple form that you can use for your records to determine fee payments to IYSA
Guest Player Form Guest Player Form  (IYSA does not need a copy of this, just keep it for your records). This is used when borrowing players from another club.
Player add/drop Form For adding or dropping players on a team
Transfer Form For transfering a player from one club to another
Internal Club Rec List For submitting an internal league list of players
Medical Release An example of a Medical Release, you can use your own too if you wish
Permission to Play in another State For a player that is registering for an Idaho club that resides in another State and visa versa
Application to Host a Tournament Application to host a tournament
International Hosting Agreement If you want International teams at your event you will need this
International Travel For teams wishing to compete in an event outside of the USA


INSURANCE INFORMATION

ACCIDENT / MEDICAL INSURANCE (Secondary Coverage  Only)

The insurance policy is for major medical only and carries a $2000 deductible

You have 90 days from date of injury to submit claim form.
For claims to be eligible for coverage you must seek medical attention within 60 days from date of injury.
For claims to be eligible you must have your own primary coverage

Procedure to follow.


1.   Either fill in online or manually complete a blank claim form (see above link). Please note that filling in an online form does not allow you to submit the claim, it simply allows you to fill it out neatly as if it was typed. You still need to print the completed online form

2.   Submit the form to IYSA. DO NOT SUBMIT BILLS. we will verify the claim and that the player is IYSA registered and that the event at which the injury occurred is sanctioned by IYSA and US  Youth Soccer

3.    IYSA will submit the form to the insurance company

4.    The insurance company will contact you, at which time they will issue you a claim number and you can then submit the bills to the.
